With soaps, mannequins are used daily on set now, so it was convenient to use one to visually represent the battlefield raging in Thomas’ mind.

When Thomas first began having auditory hallucinations, he knew that a talking mannequin wasn’t possible. He tries to get a grip on his mental state. However, he is struggling more and more each day. The hallucinations are happening more frequently, for longer periods of time, and are even occurring when Thomas is away from the doll.
He’s losing the war between sickness and sanity. The two cannot co-exist. Now, we have seen Thomas have periods of hearing voices, then having stretches of time when he’s “normal.”
However, we will see that dissipate as the mental illness gets a stronger foothold. Eventually, Thomas will reach the point of no return.

Without treatment, the voices will turn into visual hallucinations, and then the delusions will take hold. After that, Thomas will have what is called a “break-in reality,” Thomas will no longer be “present.” It’s like someone else takes over (think Invasion of the Body Snatchers), but you have no clue what happened because that file in your brain’s memory was “deleted,” kind of like an extended blackout.
Now, Atkinson told Soap Opera Digest that Thomas is trying his best to keep his head above water. However, he doesn’t realize that he is already in way too deep. It sounds like things are going to get much worse for Thomas before they get better.
