The fan speculation is that Thomas will be diagnosed with a brain tumor and it could explain his wacky behavior. However, no legitimate spoiler site has reported any tumor storyline yet.

If the B&&B writers do go that route, viewers can see Hope feeling guilty about getting an annulment. She might feel like Thomas wouldn’t have kept the Beth secret if he didn’t have something wrong with his brain.
Assuming that’s the route they are going, she could assume after surgery he would be a good husband. The problem is if that’s where the storyline is going, where will that leave Liam Spencer (Scott Clifton)?
