As far as fans know, he only had three … so, who is this “fourth” child? Well, let’s talk about what we know.
Cesar Faison’s Family Tree
General Hospital spoilers remind fans that, as far as viewers are concerned, Cesar had three children: Britt, Nathan West (Ryan Paevey), and Peter August (Wes Rasmey).
This is why it was so confusing that Britt mentioned a “fourth” child of Cesar’s. Now, there’s a small chance that WSB Director Ross Cullum (Andrew Hawkes) could be an older child of Faison’s.
After all, he does look a lot like the evil genius and shares the vile qualities of the man, as well. Still, all arrows are pointing toward another theory.
Nathan West Had An Unknown Twin

From the moment Nathan returned to Port Charles, his sister Britt – who should’ve been over the moon when she realized her brother was alive – was horrified.
Britt wanted him nowhere near his son, James West (Gary James Fuller), initially, and she was (and still is) very wary of Nathan since his arrival.
In fact, the two haven’t re-bonded at all since Nathan’s comeback to the land of the living; they spend minimal time together, all things considered.
Could it be that “Nathan” is actually Nathan’s twin brother? GH viewers can’t help but feel as if something is very “off” about Nathan since his return.
He’s messed up at work when it comes to routine evidence protocol, he slipped up about his father in front of Lulu Spencer (Alexa Havins Breuning), he needed major practice when it came to his baseball ‘swing’ (something he used to be a pro at); plus, it’s hard to shake the fact that Maxie Jones (Kirsten Storms) noticed something “off” about her thought-to-be-dead husband.

This Is A Major Clue
Britt’s slip wasn’t by accident; noting Faison had four kids has to be a major clue regarding a plot twist on the horizon.
She’s deep into his final project, so perhaps she’s in the loop that Nathan had a twin, even if it might not be known by Liesl Obrecht (Kathleen Gati).
After all, the extra baby could’ve been swiped during delivery, which is a routine explanation as to how unknown twins pop up.
It could also explain why Nathan’s DNA tests and bloodwork matched the man, who looks a lot like Nathan, but seems nothing “like” him.
